Appointment with Surgeon

After setting the date for surgery, a 10% deposit of total surgery price is required to secure the date.

The following are required papers that you can send to us by e-mail scan at info@thaisurgerytravel.com or through our fax number (662) 714-8649;

1. Medical check up for blood test; CBC, HIV Electrolytes, FBS, Creatinine Urinalysis, Alkaline Phosphatase, Chest X-ray and SGOT LDH EKG
2. Photo identification such as a driver’s license or a passport
3. A copy of claimed diagnosis or referrals.  Please bring an original signed with you so that the doctor can collect it during surgical interview.
