Breast implant is to enlarge
small or underdeveloped breasts, or to reshape breasts that
that are out of shape and fullness due to pregnancy, nursing
or weight loss. It is also to balance breasts of different
sizes and shapes as well as to improve personal satisfaction.
Surgical Procedure
The method of inserting and
positioning your implant depends on individual anatomy. The
incision can be made either in the crease where the breast
meets the chest, around the areola (the dark skin surrounding
the nipple), or in the armpit. Every attempt will be made
to assure that the resulting of scars will be almost invisible.
Type of Implants
All breast implants utilize
a silicone shell but contain different kinds of filler, silicone
gel or saline. The outer silicone shell may be smooth shiny
and polished, or a slightly rough texture. Breasts implanted
with the textured implants are less likely to develop contracting
capsules than those with the smooth implants.
Placement of Implants
There are two techniques for
placing implants.
1. The implant is placed under the breast tissue and over
the chest muscle. That is, your breast tissue
and skin will be lifted to create a pocket, either directly
behind it or underneath the chest wall muscle. The implants
will be placed in the center beneath your nipples.
2. The implant is placed under the chest muscle and breast
tissue. Some case the implant is needed to be placed
behind your chest muscle to reduce the potential capsular
contracture. It produces less interference with breast
examination by mammogram than the direct placement of the
implant behind the breast tissue. However, this method
can be more painful than the method of direct placement under
the breast tissue.
These two options will be thoroughly discussed between the
patient and the surgeon before surgery.
Possible Risks
Capsular contracture will occur
if the capsules around the implants begin to tighten and thicken.
As a result, the breasts feel hard and appear unnatural.
Hard breasts can be treated in several ways. Sometimes a removal
of the capsule is needed, or perhaps the implant needs to
be replaced. Bleeding is usually minimal during the operation.
In an unusual case, bleeding after surgery may cause a large
blood clot that needs to be removed. Sometimes nipples
can become oversensitive, under sensitive, or even numb. These
symptoms usually disappear within time.
Infection is an uncommon risk. In case it is infected,
the implant must be removed immediately. You need to
wait for several months before repeating the operation. There
might be a chance of an occasional break or leak of implants,
if a saline implant breaks, the implant will deflate in a
few hours and the salt water will be harmlessly absorbed by
the body. If a break occurs in a gel implant, however, one
of two things may happen; one, If the shell breaks but the
scar capsule around the implant does not, there is no detect
any change. Two, if the scar breaks or tears, especially following
severe pressure, silicone gel may move into surrounding tissue.
The gel may collect in the breast and cause a new scar to
form around it. This may require a second operation and replacement
of the leaking implant. In some cases, it may not be possible
to remove all of the silicone gel in the breast tissue if
a rupture should occur but the new type of gel so call Cohesive
silicone gel will not cause this problem very often.
Side Effects
The severity and duration of
side effects may vary, particularly when another cosmetic
procedure was performed at the same time.
After Care
All dressings and bandages
are removed on the third day following the operation. Afterwards,
you are able to bath as usual. However, you need to
gently massage the implants upward, downward, and inward in
order to decrease the chances of capsule contracture. An aggressive
manipulation of the implant or strong squeezing can possibly
cause an inflammation, producing an adverse effect on the
capsules.
During the first month following surgery, you should exercise
your implants three to four times per for fifteen to twenty
minutes. After that, if breast contour improve satisfactorily,
you will need to exercise only once or twice daily.
Routine mammograms should be continued after breast augmentation
for those who are in the appropriate age group, when request
a routine mammogram, be sure to go to a radiology center where
technicians are experienced in the x-ray of a breast with
an implant. Additional views will be required. Ultrasound
examinations may be of benefit in some cases with implants
to detect breast lumps or to evaluate the implant.
Recovery Period
You need to stay 7 days in
Bangkok for post operative care.
Post Operative Care
You should keep your arm from
wide extension as much as possible for at least 3-5 days after
surgery. After removing the gauze dressing, you may
be required to wear a support bra to help your breasts form
and maintain a balancing shape. You may do some exercises
and return to normal physical activities within a month or
two when soreness has subsided.
